Filing on Behalf of the Employee

If the employee is incapacitated, someone may take action on his or her behalf, including a family member, union official, or representative, by completing the paper form version of the claim form and providing it to the Bureau Workers’ Compensation Specialist or the MCA Claims Specialist for entry into ECOMP.  The supervisor may give such notice as well. The form must contain the signature and date of the person giving notice. The employee or their representative should ensure that medical documentation and/or other information is provided to support the claim.
